Rates & Insurance

NMK & Associates is currently paneled with most PPO insurance plans, including Blue Cross Blue Shield PPO, Aetna PPO, Cigna PPO, Magellan (not EAP plans), United Healthcare and Self-Pay rates apply for those who prefer to not utilize insurance or if we are an out-of-network provider. When utilizing your PPO plan, your out-of-pocket cost will be determined by your insurance plan; please verify your coverage and benefit level. Self-Pay Rates are published on Schedulicity, our online booking calendar for new clients to use. Please click the button “Schedule Now,” on the “Contact,” tab from the “Home,” page to book appointments, view services, and providers’ availability. Once you have booked an appointment, the therapist selected will contact you and ask you to fill out the forms necessary, via our website to begin sessions.

We can provide an invoice for services rendered, which will allow you to submit claims to your insurance provider if your plan is out-of-network. Please note that the majority of insurance plans will offer you a reimbursement of some of your out-of-pocket expense per session.

A sliding scale option may be available with some providers if you are seeking a reduced fee due to financial hardship. Alternatively, shorter appointments or less frequent sessions may help reduce financial concern. Consult with your therapist to come up with a treatment plan that works; we sincerely want to help and we work to ensure access for as many people as we can.

FAQ’s Regarding Insurance

Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

  • What are my mental health benefits via my healthcare insurance plan?
  • What is my deductible and has it been met?
  • How many sessions per calendar year does my plan cover?
  • How much does my plan cover for an out-of-network provider?
  • What is the coverage amount per therapy session?
  • Do I need an authorization number from my insurance carrier?
  • Does my employer have an account for healthcare benefits that covers services?

Means of Payment

Major credit cards (Visa/MC/Discover/AMEX) are accepted for payment via our Schedulicity app.
Note: there is a $4.00 service charge, per hour session, when you use a credit card.

Cancellation Policy & Fee

If you would like to cancel or re-schedule your appointment, please modify your appointment on Schedulicity and let your therapist know, 2 business days prior to the appointment. Late cancellations and missed appointments are charged a $130 flat rate for a 45-60 minute session. We’re happy to work with clients to reschedule within the same week if a scheduling conflict occurs and we can offer an alternate time.

Termination Policy

We request a minimum of two weeks’ notice before the termination of therapy. We feel this period is essential for proper closure of the therapeutic relationship for both the client(s) and therapist.

No Court Policy

If you ever become involved in a divorce or custody dispute, or any other legal matter, we will not provide evaluations or expert testimony in court. You will be asked for your signature to indicate your agreement with this provision. In the event, that we are mandated for a court matter, as required by law, to produce documents or information, to appear in court or in a deposition, on your behalf—as your provider, we will charge a fee, including, but not limited to, any travel, preparation or consultation time. Fees for our time will range from $200-350 per hour, depending on the request, level of attention, and complexity. If any entity requesting materials or time is unwilling to pay for costs, as the client, you will be responsible for this fee. If you are seeking a custody evaluation, you may be better served to find a Psychologist or Guardian Ad Litem (GAL), outside of our practice.
