If you are a first-time client, please fill out the Intake, Informed Consent & Credit Card Authorization forms, and email to your provider before your first appointment. We’d appreciate you filling out the Clinical Intake Form; if you’d like to wait until after your first appointment, that’s fine. Thank You.
If you would like us to coordinate with another provider (e.g. psychiatrist, endocrinologist, etc.), please complete the Consent to Release form and email your provider.
